Job Summary

Provides basic nursing care tasks for designated patients under the direction and supervision of the Registered Nurse.

Essential Functions

  • Recognizes the importance of patient/staff safety precautions as it relates to their specific department/area of responsibility. Consistently utilizes precautions and follows appropriate procedures to enhance patient/staff safety in their daily operations. Identifies and reports any potentially dangerous situations to their manager which could cause medical error/patient or staff harm. Immediately takes action, where appropriate, to minimize the risk of injury. Where appropriate to staff member’s responsibilities, reports errors, when these occur, to their supervisor and on an occurrence report. Participates, when appropriate, in Root Cause Analyses and/or other efforts to reduce risk of medical error/patient or staff harm.
  • Performance Improvement
  • Participates in performance/customer service improvement; actively assists in improving performance based on results obtained from customer satisfaction/PI activities. Recognizes systems approach to PI. Where appropriate, participates in Performance Improvement activities including team membership. Identifies processes that need improvement.
  • Patient Care – Provides patient with assistance with their personal hygiene based on the needs of the patient and the assignment by the R.N.
  • Bathes, provides skin care, back rubs and mouth care on all assigned patients.
  • Accurately takes and documents temperature, pulse, respiration and blood pressure on all assigned patients.
  • Weights patient upon admission and every seven days.
  • Accurately uses glucometer (follows policy and procedure.
  • Maintains specific isolation precautions, practices both medical sepsis and sterile technique.
  • Assists with admissions transfers, discharge process, organizes patient belongings and ensures that they accompany patient upon discharge or transfer.
  • Performs skin care protocol as per policy and reports all changes to registered nurse.
  • Demonstrates ability to apply and care for the patient with a sequential compression device.
  • Accurately performs: colostomy care - stoma and skin care and application of appliance, enema, toileting rounds as per procedure, set up and care for patient with hypo/hyperthermic machine, sets up and care for patient with sitz bath, empties and records Foley drainage, Hemovac and Jackson-Pratt drains and suction canisters, care of the restraint patient, q30 minute observation with release with in time frame of policy, documents all interventions
  • Accurately documents tasks on flow sheet.
  • Documents Temp, Pulse, Respiration, and Blood Pressure on appropriate forms
  • Documents intake and output on assigned patients utilizing both the bedside sheet and the cumulative 24-hour sheet.
  • Maintains current glucose monitoring certification
  • Documents glucose results in all designated places per policy.
  • Takes report on all assigned patients at the beginning of each shift from RN and reports all significant patient related issues including, but not limited to abnormal or unusual vital signs, when patient safety is at risk, when there are any signs of redness.
  • Maintains patient rooms in a clear orderly manner.
  • Serves fresh ice water during the assigned shift and as requested.
  • Maintains patient bedside with necessary supplies.
  • Answers patient call lights and requests promptly.
  • Ensures bedside stand and over-bed tables are neat and clean.
  • Empties bedpans, urinals and commodes cleans them and puts them away after use.
  • Assists patients with their meals by opening containers, cutting up food and placing food with in easy reach of the patient.
  • Maintains break room, lounges and public areas in a neat orderly fashion.
  • Is proficient in demonstrating the procedure for disaster plans and protocols
  • Demonstrates proficiency when called upon to both life safety/emergency preparedness situations.
  • Uses proper body mechanics when moving or transporting patients and handling equipment.
  • Actively supports and adheres to all safety and infection control guidelines and procedures within the department.
  • Demonstrates knowledge and integration of OSHA Blood-borne Pathogen and adheres to all safety handling of hazardous materials.
  • Demonstrates age appropriate communication skills during patient interaction.
  • Gives specific consideration of patient’s age related needs during processes of assessment, treatment, and care.
  • Demonstrates knowledge of human growth and development in patient care management.
  • Performs other duties as requested and required.

Qualifications

  • High School Diploma or equivalent required
  • One year experience as a Nursing Assistant in a health care facility or verifiable home care setting or current Nursing Assistant Certification.
  • Ability to read, write and communicate in English
  • Successful completion of a Nurses Aide Program (or equivalent preparation)
  • Current Nursing Assistant Certification Preferred
  • Nonviolent Crisis Intervention Training on a Bi-Annual Basis Required
