Part Time Project Coordinator- Aging and Disability Resource Center

Examples of duties

Under administrative direction, is responsible for planning and administering a specific project or program for a City department. Serves as the administrative supervisor on projects or programs. Plans, assigns, supervises, and reviews activities of subordinate personnel. Monitors the project or program's operating budget and assists in the annual preparation of same. Designs, implements, and monitors procedures utilized in program evaluation. Develops and secures commitments from other City departments and agencies to provide services to program participants. Provides technical assistance or reviews program policies and procedures. Maintains current information and oversees the preparation of project status reports. Prepares financial and narrative reports and program applications. Communicates project/program status and information to municipal officials and/or public and private entities. May supervise employees assigned to the respective project. Performs other related duties as required.

Qualifications

Bachelor's Degree from an accredited four-year college or university in Business or Public Administration, Social Sciences, or closely related field preferred. Two (2) years of any equivalent combination of education, training and experience in program administration required. Must be able to lift and carry thirty (30) pounds. Valid State of Ohio Driver's License required.

Additional Essential Duties and Responsibilities:

  • Provide direct, person-centered assistance to older adults and individuals with disabilities based on identified needs.
  • Conduct outbound cold calls prospective and existing clients to introduce ADRC services, identify unmet needs, determine eligibility or interest, and establish direct client contacts.
  • Communicate with family members, caregivers, or designated representatives when a client is unable to participate directly, as appropriate and permitted.
  • Conduct home visits and/or in-office appointments with clients to provide information, complete forms, assist with applications, make referrals, and connect clients to appropriate resources.
  • Accompany clients to appointments or service-related activities when appropriate and within the scope of the program.
  • Provide direct assistance to clients at community outreach events, including completing applications, forms, referrals, resource requests, and other service-related documents with clients.
  • Assist clients in navigating public and community-based resources related to aging, disability, healthcare, housing, transportation, food assistance, utilities, benefits, and other supportive services.
  • Follow up directly with clients regarding referrals, applications, appointments, service connections, and unresolved needs.
  • Identify barriers preventing clients from accessing services and assist in developing appropriate solutions and maintain ongoing communication when additional assistance or follow-up is necessary.
  • Develop and maintain relationships with community agencies and service providers to facilitate appropriate client referrals and service connections.
  • Accurately document all qualifying direct-service, client information, units of service and other required information into designated reporting databases in accordance with departmental procedures and contract requirements.
  • Monitor individual progress toward assigned service goals and communicate challenges or opportunities to the ADRC manager.
  • Work collaboratively with ADRC staff to identify opportunities to provide appropriate direct services to clients and meet program deliverables.
  • Protect client confidentiality and handle sensitive personal information in accordance with City policies and applicable laws and regulations.
