Job Duties

Available shifts include: 7:00am-3:00pm, 3:00pm-11:00pm and 11:00pm-7:00am.

2 days a week and every other weekend (holidays as needed).

AMAZING SHIFT DIFFERENTIAL PAY
2nd Shift (3pm—11pm): $3.00
3rd Shift (11pm—7am): $2.00
Weekends: additional $1.00 + shift differential

We are seeking a passionate and dedicated Licensed Practical Nurse who is committed to join our team and provide exceptional and compassionate care to residents in a long-term care/skilled nursing environment. This role will be responsible for delivering high‑quality nursing care, administering medications, monitoring resident conditions, and support the daily clinical needs of our residents.

Responsibilities include, but are not limited to:
•Provide direct nursing care to residents under the supervision of an RN and within the scope of LPN practice.
•Administer medications and treatments accurately and in accordance with facility policy.
•Participate in admission, discharge, and transfer processes.
•Work in partnership with nursing Admins team, physicians, therapists, CNAs, and other team members to deliver comprehensive care.
•Perform resident assessments and promptly report changes in condition to the supervising nurse or provider.
•Document all care, assessments, treatments, and observations in accordance with regulatory and facility standards in the Point Click Care system.
•Assist with care planning and ensure care is delivered according to individualized plans.
•Provide wound care, and other nursing procedures as needed.
•Maintain infection control standards and promote a clean, safe, and respectful environment.
•Communicate professionally with residents, families, and staff while showing patience and empathy.
•Support and guide CNAs to ensure quality resident care and adherence to policies.

Minimum Qualifications

•Must have a current Licensed Practical Nurse license in the state of Virginia. CPR certification required upon hire or willing to obtain.
•Ability to work assigned shifts, including weekends and holidays as needed.
•Knowledge of long-term care practices, resident rights, and basic nursing procedures.
•Strong communication, assessment, and documentation skills.
•Ability to perform physical tasks such as lifting, transferring, turning, and assisting residents with mobility.
•Basic knowledge of personal care practices, infection control, and safety procedures.
•Ability to observe and report changes in resident condition promptly and clearly.
•Ability to document care activities clearly, accurately, and in compliance with facility standards.

Additional Considerations

Previous nursing home, long‑term care, or geriatric experience preferred but not required.
