Location: Early Childhood

10-month schedule, hours vary (part-time, some night and weekend hours required)

Status: Non-Exempt

Starting Pay: $29.75/hour

Essential Functions

  • Develops and maintains a community resource network for the purpose of linking families with needed community

supports.

  • Maintains and keeps the family caseload up to date, using the necessary documents for the purpose of ensuring

visit requirements for each family are met and the number of visits and hour goals outlined in the contracted level

are achieved.

  • Maintains and keeps to date your family caseload, using the necessary documents for the purpose of ensuring you

  • Maintains the confidentiality of all families and the services they receive for the purpose of ensuring their privacy

and trust.

  • Participates in a variety of meetings, professional development, and committees (e.g. staff meetings, PLC

meetings, individual reflective meetings and other required professional development, etc.) for the purpose of

conveying and/or gathering information required to perform functions, and remaining knowledgeable about

program guidelines.

  • Participates in meetings, workshops and seminars (e.g. IEP's, etc.) for the purpose of conveying and/or gathering

information required to perform functions.

  • Plans and facilitates family and parent group connections for the purpose of build relationships, support parent

learning, and create a community of families growing together.

  • Plans each visit using the Visit Tracker planning guide, checking in with the family, gathering materials, and

conducting the visit in their home, at the center, or through an approved virtual platform such as Google Meet for

the purpose of for the purpose of conducting a personalized home visit for each assigned family.

  • Responds to inquiries about the requirements and procedures of the program as outlined in the procedure manual

and through addendum provided via emails, agendas, and the website (e.g. procedure manual, addenda, emails,

agendas, and the website, etc.) for the purpose of ensuring consistent and accurate implementation of program

expectations.

  • Responds to potential emergencies encountered during home visits for the purpose of ensuring safety of students

and/or personnel.

  • Serves Parkway's mission, vision, and ethos by consistently demonstrating its values and goals for the purpose of

fostering alignment and shared commitment across our district.

  • Documents the visit in Visit Tracker as well as perform all follow-up tasks necessary within 48 hours for the

purpose of ensuring accurate records and continuity of support for each family.
