Job Description

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. We are seeking a Palliative Care Coordinator will provide administrative support to the Palliative Care and Pediatric team. The role serves as the Palliative Care Coordinator in accordance with BTBH organizational policies and procedures and applicable laws and regulations.

Schedule:

Monday - Friday; 8:30am - 5:00pm

Location:

Onsite; Larkspur, CA with occasional coverage to our Sonoma, CA office

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

  • In collaboration with and under the direction of the Palliative Care Manager, run EPIC reports to collect and track metrics in alignment with the program goals and needs.
  • Perform MATCH audits and outreach to MATCH patients at the direction of the Program Manager
  • Coordinate and schedule clinician visits based on contract obligations and Epic reporting.
  • Under the direction of the Palliative Care Manager, partner with contracted vendors, hospitals, and facilities to facilitate strong referral pathways, provide education about programs and team approach.

Education/Experience

  • Associate degree (A. A.) or equivalent from two-Year College or technical school; two years related experience and/or training; or equivalent combination of education and experience.
  • Minimum two years’ experience providing administrative support to a mid-level or senior manager.

Pay Range:
$25.00 - $35.00 hourly
