Established in 2020 by the Board and executive leadership of Community Hospice & Palliative Care, Alivia Care, Inc. was created to advance a broader mission of serving more people in more places, helping individuals live better with advancing illness.
As part of this mission, Alivia Care collaborates with qualified independent clinical contractors who bring specialized expertise and professional services to the patients and communities we serve. These independent professional relationships play an important role in supporting our commitment to compassionate, high-quality care across our growing continuum of services.
The Palliative Care Advanced Practice Provider provides compassionate, patient-centered medical care to individuals living with serious or life-limiting illnesses. This role independently performs patient examinations and evaluations, develops diagnoses and treatment plans, orders and interprets diagnostic testing, prescribes medications and treatments, and provides ongoing clinical follow-up within the scope of professional practice. Working collaboratively with physicians and the interdisciplinary care team, the provider supports patients and families in clarifying goals of care and making informed healthcare decisions. The provider maintains timely and accurate clinical documentation while ensuring care is delivered in accordance with applicable legal, ethical, professional, and organizational standards.

Opportunities are intended for qualified independent clinical professionals interested in providing services to Alivia Care under an independent contractor agreement. Any engagement will be governed by the terms of a mutually executed professional services agreement and applicable legal and regulatory requirements.
Independent contractors must maintain all licenses, certifications, credentials, and other qualifications applicable to the services they provide. Where required by law, engagements are subject to applicable background screening, credential verification, and other regulatory requirements.
This posting is a solicitation for independent professional services and does not constitute an offer or guarantee of employment or contractual engagement. The nature and terms of any engagement will be established through a separate written agreement.
