Assist with day-to-day operations of the Journey and Palliative program, which are designed to manage patients who are at any stage of a chronic or life-limiting illness as they continue to seek curative or palliative treatment. Support the transition of patients to the correct level of care, as decided by patient and family.

Essential Duties and Responsibilities

include the following. Other duties may be assigned as necessary.

  • Coordinate, schedule, and assign Journey or Palliative patients to providers
  • Conduct follow-up calls to patients and families to collect data about patients/family response to the treatment plan established by the assigned provider
  • Serve as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources
  • Manage referrals, when needed, to appropriate agencies required to assist the client in achieving the goals and objectives identified in their Care Plan
  • Assist patients in problem solving potential issues related to the health care system, as well as financial or social barriers (e.g., request interpreters, transportation services, or prescription assistance as appropriate)
  • Assist with medical records
  • Demonstrate excellence in both internal and external customer service
  • Understand and effectively communicate HIPAA compliance, corporate compliance, and client confidentiality
  • Ensure and/or remain in compliance with local, state, and federal regulation
  • Comply with Company’s Core Values and Core Competencies
  • Perform other duties as assigned by supervisor

Qualification Requirements

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • A minimum of 2 (two) years of experience in Palliative, Hospice, or Home Care preferred
  • Bachelor’s degree in a health-related field preferred
  • Medical records experience preferred
  • HCHB experience strongly preferred
  • Strong written and verbal communication skills
  • Teamwork orientation
  • Able to take and follow through with delegated tasks and accountability
  • Resourcefulness in problem solving
  • Proven data analysis skills with previous use of an electronic health record
  • Proven organizational skills and ability to prioritize work to meet the needs of the agency
  • Intermediate proficiency in Microsoft applications: Excel, Word, Outlook, PowerPoint
  • Ability to provide excellent customer service and demonstrate strong cultural competency
  • Ability to adhere to strict confidentiality guidelines

Certificates, Licenses, Registrations

Must have and maintain in good standing professional license, certificate, or registration, as applicable.
