Qualifications: Job Summary:

The PACE RN Care Coordinator coordinates and delivers medical care to Valir PACE Foundation participants. The RN works jointly with physicians, APRNs, other nurses, therapy, activities, dietary, social services and transportation as part of the Interdisciplinary Team to determine and monitor the care plan for each PACE participant in compliance with all national and state regulations/laws.

Duties/Responsibilities

  • Performs comprehensive exams on all participants every six months and upon change of status in accordance with CMS regulations.
  • Creates and maintains care plan for participants on case load.
  • Participates in acute care visits for participants in PACE clinic.
  • Engage the participants support system in medical care.
  • Inputs orders and authorizations as needed for meds, services and equipment.
  • Manages care and medication compliance for participants on case load.
  • Collaborates with IDT for effective and timely interventions for participants.
  • Provides hands-on care for skilled nursing treatments for participants on caseload.
  • Triage and prioritize needs throughout day and address as quickly as participants needs indicate
  • Utilizes good mannerisms and professional demeanor with participants, families, community care providers and PACE team.
  • Ensures positive health outcomes for participants through ongoing participant and support system education, medication management, care coordination, and IDT collaboration.
  • Maintains accurate and timely patient records.
  • Other duties as assigned.

Required Skills/Abilities

  • A strong knowledge of the Federal and State laws and regulations governing health care
  • Detailed understanding of government requirements/expectations for health care compliance programs
  • Strong oral and written communication, planning, and problem solving skills
  • Maintains a high degree of credibility, independence, integrity, confidentiality, and trust
  • Able to operate successfully in a constantly changing, fast-paced environment
  • Demonstrates initiative, self-motivation, practical learning skills, enthusiasm, and an ability to complete multiple tasks in a timely and accurate manner;
  • Competent and efficient on EMR and other computer programs
  • Knows and adheres to the values and goals of the Valir PACE Program
  • Maintains a safe environment for participants, visitors and staff
  • Keeps confidentiality of client records, report, and discussion
  • Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload in order to fulfill responsibilities

Education, Licenses, Certifications and Experience

  • Current Oklahoma RN License without restrictions
  • CPR Certification
  • ACLS Preferred

  • Ability to speak, hear, and read English.
  • Lift up to 50lbs. from floor to 36 (occasional)
  • Walk up to .25 miles (frequent)
  • Sustained forward bending (occasional)
  • Push up to 75 lbs of force at 36 (frequent)
  • Ability to stand (frequent), sit (frequent), sustained squat (occasional), kneeling (occasional)
  • Full body range of motion (constant)
  • Front carry two handed carry of 25lbs. up to 15 feet (occasional)
  • Balance (continuous) in walking, standing, carrying, moving patients, and equipment, etc.
