Position SummaryJob

Summary:
The PACE Intake Registered Nurse is responsible for conducting comprehensive in-home assessments to determine medical eligibility for the PACE program. This field-based role requires regular travel to prospective participants’ homes and occasional visits to healthcare facilities. The nurse will also collaborate closely with other intake team members and support additional nursing functions as needed.
Education, Licenses, Certifications and

Experience:
- Current Registered Nurse (RN) license in good standing.
- Current CPR Certification.
- UCAT Certification preferred.
- Previous experience with in-home assessments or community nursing strongly preferred.
- Experience in geriatric, home health, or PACE model care is a plus.
