About the role

Conexiones ATS/CSS is a 32-bed facility that provides clinical stabilization services to adults struggling with drugs, including heroin and prescription drugs, or alcohol. Acute treatment services are designated for up to 12 individuals for medically managed withdrawal services. At Casa Esperanza Conexiones, located at the Tewksbury Hospital, we provide residential substance abuse treatment for adults in a physically and psychologically safe environment and additional inpatient medically managed detoxification services, including those with co-occurring mental health issues. Our highly structured environment focuses on resolving addiction issues, building the foundation for a sustainable recovery, and developing a healthy, productive, and meaningful lifestyle.

The program is staffed by highly skilled professionals who work with each patient to develop an individualized treatment plan and determine appropriate aftercare. We are currently seeking a Registered Nurse and/or Licensed Practical Nurse to oversee the Medical Services at our Tewksbury Conexiones through Casa Esperanza’s Integrated Care Model, implementing doctor’s orders, monitoring specified medication/refills, monitoring and recording vital signs as needed, and maintaining medical/nursing records. The Nurse will also observe patients and provide accurate and timely documentation of their treatment progress and coordinate and assist case managers regarding clients’ progress and treatment.

What you'll do

  • Complete Nursing Assessments, COWS, CIWA Assessments upon admission
  • Complete COWS/CIWA and ASAM assessments aligned with medical orders for protocols associated to withdrawal management
  • Complete medical clearance exam at time of intake
  • Complete Medication Monitoring and all required documentation
  • Complete Medication Counts at start and finish of shift of all control and non-control substances
  • Work closely with the clinical team during MDT meetings to offer and provide client medical insight as part of case presentations
  • Consult with staff and attend case conferences, as needed
  • Work collaboratively with the medical director for the medication reconciliation process.
  • Works to achieve standing orders and medical orders for medication
  • Able to organize and keep record of frequency and urgent medical exams
  • Directly works in collaboration with community providers for medication refills and reconciliation.
  • Complete evaluations based on client request for medical attention, emergency and non-emergency triage and physical/mental health rounds for safety and condition monitoring
  • Keep electronic health records up to date, including progress notes and assessments of service.
  • Educating clients on their current medical diagnosis, needs and medical histories; educating clients on medication information and usages
  • Alerting physicians when conditions worsen or if abnormalities arise
  • Works in collaboration with counselors to ensure proper medication management and medication planning
  • Works directly with the program director on any medication errors and medication monitoring procedures
  • Works uniformly with the recovery specialist on medication monitoring procedure
  • Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled

Qualifications

  • Massachusetts Licensed Registered Nurse
  • Experience:
    • Nursing Experience: 2 years, required
    • Medication monitoring: 1 year, required
    • EHR systems, 2 years required
  • Spanish Bilingual Skills required
  • Ensure patient confidentiality in accordance with all federal, state, and local laws and regulations
  • Knowledge on best practices associated from the American Society of Addiction Medicine
  • Knowledge on medication-assisted treatment for substance use disorders
  • Knowledge of Substance Use Disorder and other co-occurring conditions

Physical & Mental Requirements

While performing the duties of this job, the employee is regularly required to walk, stoop, kneel, crouch, or crawl and talk or hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include distance vision, peripheral vision, and depth perception.

Schedule

Part-Time/Per Diem

Base Pay: $50/hr

Shift Differentials:

2nd shift: +$2/hr

Overnight Shift: +$3/hr

Weekend Shift: +$2/hr

Our services:

Residential Recovery: Casa Esperanza's Men’s Program serves 29 adult men and Latinas y Niños serves 20 adult women and can also house up to 12 of children age ten or younger so clients in recovery may maintain or regain custody while still in treatment. Residential services usually last six to twelve months and offer evidence-based treatment methodologies as well as assisting clients with setting long-term goals for maintaining their recovery as they transition to independent living.

Structured Outpatient Addictions Program (SOAP): Familias Unidas Outpatient Services is a licensed, CARF-accredited clinic that provides client-centered, integrated services to support the complex, long-term needs of people with co-occurring disorders. Familias is co-located with primary care services and is the only Spanish-language SOAP in Greater Boston.

Supportive Housing: Nueva Vida provides15 single-occupancy units, Dunmore Place & 300 Eustis Street provides eight family units, and Nueva Esperanza provides 14 individual studio units for a total of 37 units of permanent safe, sober housing for individuals and families, with on-site support for maintaining recovery and building independent living skills.

Clinical Stabilization: Conexiones is a structured 24-hour inpatient clinical stabilization program for individuals early in their recovery which includes nursing care, individual and group therapy, psycho-educational groups, case management, health education, and aftercare planning.
