The Outreach Supervisor is responsible for managing and supervisingthe team of Outreach Specialists, and a Community Support Navigator. The incumbentwill individually monitor the team’s performance and productivity, so the team meets the daily and monthly target. She will review and audit all consents to ensure that the team maintains the health home standards. The Outreach supervisor will manage the workflow, create schedules and delegate tasks as required. Also, the Outreach Supervisor will assess the work performance of the outreach specialists and document areas that require improvement.
Provides ongoing supervision to the team of Outreach Specialists and Community Support Navigators. • Performs periodic performance reviews of the assigned team members to provide guidance and support. • Observes staff performance during house calls, office and specialty clinic hours and provides ongoing feedback to improve the outreach specialist’sefficacy. • Utilizes internal project management instruments to monitor individual and team performance in meeting daily and monthly deliverables. • Ensures that the teams members meet the daily targets and monthly deliverable. • Works with the enrollment supervisor to monitor the outreach and enrollment process for each patient. • Ensures that the assigned team members receive ongoing training to increase their efficacies to perform their duties. • Monitor enrollment process for each patient from outreach to enrollment.• Reviews and approves all new enrollments to determine the appropriateness of their eligibilities. Also, ensure that the service linkages are developed and maintained.• Collaborates and coordinates with internal and external stake holders, including house calls, office and specialty offices and CBO providers to increase Essen’s visibility and health home enrollment. • Collaborates with the House call division, medical and specialty office and CBO to increase referrals. • Serves as the point of contact for Lead Health Homes regarding enrollment.• Works collaboratively with other outreach teams to ensure Health Homes goals and objectives are being met and conducts quality reviews and patient satisfaction to ensure that outreach and enrollment workflows are being followed. • Follows best practice and clinical standards and adheres to departmental and State guidelines.• Performs all other duties or actions as required.
Associate’s/Bachelor’s/master's degree preferred.• At least two years’ experience in outreach. • At least one year of supervisory and training experience is preferred. • Excellent written and oral communication skills required.• Proficiency in Spanish and other languages is preferred. • Ability to multi-task well while maintaining a positive “can do” attitude.• Demonstrated ability to manage a team in a fast-paced environment while building and enhancing team productivity.• Demonstrated professionalism and leadership skills along with the ability to develop, direct and support staff.
