Job Description

LOCATION:Herberger Center – Phoenix, Arizona

STATUS:FTE/40 hours/wk

MISSIONSTATEMENT:

QUALIFICATIONS:

  • Associate of Arts Degree in Human Service or equivalent combination of education, training, and experience
  • Ability to work independently with limited supervision, but also coordinate services with other team members
  • Ability to relate to people in crisis, who suffer from serious mental Illness, chemical dependency, physical disability, chronic homelessness, or diverse cultures
  • Comprehensive knowledge of community resources
  • Excellent verbal and written communication skills
  • Proficient in MS Office Suite
  • Pass a criminal background check and have a good driving record
  • Must possess a current Arizona Driver's License
  • Bilingual (English/Spanish) preferred

RESPONSIBILITIES:

  • Provide outreach services to areas of Phoenix where the homeless congregate
  • Work with clients to develop case plans and establish an action plan, availability, and eligibility for services to ensure a smooth transition through the continuum of care
  • Provide transportation for program participants as requested by TSA caseworkers
  • Contact Program Manager for direction and assistance with client cases, conflict resolution, crisis intervention, documentation issues, client advocacy, or crisis situations as needed
  • Complete all required case management documentation and data entry regarding client files, eligibility screenings, and reports
  • Be familiar with and contribute to agency compliance with expectations and grant outcome mandates
  • Maintain and build on current relationships with partnering agencies and field contacts
  • Communicate in a professional manner with cooperating community agencies
  • Perform follow-up communication with referral agency as needed
  • Inform Program Supervisor when new contacts are made to facilitate an on-going relationship
  • Attend required supervisory, team and educational meetings as arranged by the Program Manager
  • Report issues of concern, abuse, or neglect to designated agency, Program Manager, and Social Services Director immediately
  • Properly maintain Program vehicles and complete travel logs in detail
  • Promote and maintain an orderly and professional work environment
  • Perform additional duties as required

The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move up to 75 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to adjust focus. Must have the ability to operate a telephone, a desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
