
The Outreach Specialist supports the Enhanced Care Management (ECM) program and other special programs by conducting outreach, engaging and enrolling members, and providing ongoing care coordination support. This position serves as a key member of the interdisciplinary care team and assists members in accessing healthcare services, community resources, and social supports that improve overall health outcomes.
The Outreach Specialist works closely with Lead Care Managers, providers, administrators, community partners, and other interdisciplinary team members to ensure continuity of care and successful member engagement. Services may be provided telephonically, electronically, in clinic settings, in members' homes, or within the community.
This role is responsible for member outreach, enrollment, care coordination, barrier resolution, resource linkage, and maintaining accurate documentation of all patient interactions and program activities.
· Conduct outreach to eligible and referred members, explain program benefits, and facilitate enrollment into ECM and other special programs.
· Complete member enrollments and required program documentation in accordance with established guidelines.
· Maintain ongoing communication with assigned members to support engagement and continuity of care.
· Provide care coordination services, including scheduling appointments, facilitating referrals, coordinating services, and ensuring follow-up on care plan activities.
· Identify barriers to care and connect members with appropriate healthcare, social service, and community resources.
· Support Lead Care Managers by assisting with member follow-up, documentation, and coordination of services.
· Collaborate with healthcare providers, community-based organizations, social service agencies, and internal departments to address member needs.
· Conduct home visits and community visits as needed to engage members, complete enrollments, provide care coordination support, and assist with care plan goals.
· Monitor member progress and communicate significant concerns or changes in condition to the Lead Care Manager and care team.
· Maintain accurate, timely, and complete documentation of outreach efforts, member interactions, enrollments, and care coordination activities.
· Ensure compliance with all ECM program requirements, organizational policies, privacy regulations, and documentation standards.
· Perform other duties as assigned in support of ECM and special program operations.
· High school diploma or equivalent
· Spanish proficiency
· Experience in patient outreach preferred.
· Strong interpersonal and communication skills with the ability to engage patients from diverse backgrounds.
· Ability to work independently while collaborating effectively within a multidisciplinary team.
· Proficiency in Microsoft Office Suite and experience with electronic health records (EHR) systems preferred.
· Strong organizational skills and attention to detail.
