The Outreach Specialist is responsible for implementing and maintaining relationships with referral sources, community leaders, business owners, and the general public.
ESSENTIAL FUNCTIONS
Essential functions are duties, which are essential or primary to the position or the reason the position exists. An individual must be able to perform the essential functions of the position with or without reasonable accommodation.
- Coordinating with courts, probation officers and other community resources.
- Providing clients and family members with information about addiction issues and available services/programs.
- Following and reporting on the progress of discharged patients to determine the effectiveness of their treatment.
- Gathers information on individual and community needs and identifies strengths, weaknesses, needs and problems in order to formulate appropriate plans.
- Provides oversight to community members and outposts about The Counseling Center and encourages a functional relationship between the two.
- Conducts educational services for the community in an effort to educate the community on substance abuse, mental health, overdose awareness and harm reduction.
- Provides information about TCC and the full continuum of care. Encourages participation in treatment for those with substance abuse/mental health.
- Provides input to treatment team and communities regarding the developments and needs of the client population. Meets with clinical staff and Treatment Director for review, input and consultation.
- Complete 50 outreach contacts per week.
- Conduct 10-15 in person visits or meetings per week.
- Establish 2 to 3 new referral relationships per week.
SECONDARY FUNCTIONS
Secondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center, Inc.
- Assists in the evaluation of program goals and objectives.
- Performs any other duties assigned by the Director.
COMPETENCIES
- Demonstrated knowledge of alcoholism and chemical dependency.
- Demonstrated knowledge/skill of clinical practice and performance, including a thorough knowledge of diagnostic assessment, treatment planning, counseling interventions.
- Ability in the area of effective communication, interpersonal skills.
- Ability to make decisions and to demonstrate sound judgment.
- Ability to effectively manage self and tasks for maximum efficiency.
- Ability to complete all required documentation in accordance with agency policy.
- Knowledge of federal and state laws, and public and private resources pertaining to counseling services.
QualificationsMINIMUM QUALIFICATIONS, TRAINING AND EXPERIENCE
- Extensive knowledge and experience in alcohol and substance abuse treatment and mental health.
- Prior community experience with outreach, including experience working with chemically dependent people/families, experience with the 12-Step Recovery Program.
- Experience with Narcan administration.
- Valid Ohio Motor Vehicle License.
- Maintain CPR/First Aid training.
