
We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our multidisciplinary team in our outpatient diabetes clinic. This is an excellent opportunity to work with a collaborative, patient-centered group dedicated to delivering comprehensive, evidence-based care to individuals living with diabetes and related endocrine conditions.
This position is based in Keene, NH, a beautiful college town in southwestern New Hampshire with outdoor recreational opportunities, cultural venues, and a vibrant downtown.
Key Responsibilities:
Conduct patient visits and develop and implement individualized care plans focused on disease prevention, glycemic control, and long-term health
Collaborate closely with endocrinologists, diabetes educators, dietitians, and nursing staff to deliver holistic patient care
Educate patients and families on diabetes management, lifestyle modifications, and medication use
Order and interpret labs, adjust medications, and monitor treatment outcomes
Participate in quality improvement initiatives and contribute to program growth
Qualifications:
Nurse Practitioner or Physician Assistant with ability to obtain/maintain New Hampshire licensure
National certification as a PA, FNP, or AGNP
Prior experience in endocrinology, diabetes care, primary care, or related specialty
Strong clinical judgment, communication, and patient education skills
