Summary: The Outpatient Counselor is responsible for providing direct clinical services to individuals who have been affected by chemical dependency, psychiatric problems and/or co-occurring disorders.
Responsibilities and Duties:
- Functions as a case manager for patient caseload.
- Maintains clinical record including SNAP and BAM assessments, treatment plans, clinical summaries, progress notes, case consult notes, MAT notes, crisis plans, discharge summaries, and continuing care plans in accordance with established time frames.
- Audits clinical record for existing patient caseload to ensure compliance with all licensing, regulatory and accrediting agency requirements.
- Contacts and maintains communication with family, referral sources, legal systems, and/or outside providers and documents in the clinical record.
- Completes orientation with new patients and documents in the clinical record.
- Facilitates individual, group and family counseling sessions.
- Conducts educational seminars and lectures to patient population in a professional and competent manner.
- Completes substance use disorder assessments and corresponding authorizations with insurance companies when necessary.
- Completes continued stay reviews using ASAM criteria.
- Makes referral recommendations when necessary.
- Provides clinical outreach to patients for attendance to programming, case management, or bump ups to a higher level of care when necessary.
- Prepares letters for patients, ensuring they are reviewed by the program manager before being provided to patient.
- Responds to and addresses unusual or emergency situations.
- Encourages patients to understand the benefits of recommended level of care.
- Operates as part of multi-disciplinary treatment team.
- Assesses healthcare equity for current and future patient needs.
- Collects and processes urine drug screen samples from patients.
- Participates in clinical supervision, department meetings and training as required.
- Assists with coverage at other sites when necessary.
- Protects the confidentiality of patient information.
- Performs other related duties as assigned.

QualificationsEducation and

Experience:
- Master’s degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in human services) or other related field which includes a practicum in a health or human service agency preferably in a drug and alcohol setting.
- Bachelor’s degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in human services) or other related field and one-year of clinical experience in a health or human service agency, preferably in a drug and alcohol setting.
- Must hold a professional license through the PA Department of State or be credentialed through PCB at the time of hire or be actively working towards licensure or certification. Licensure or certification is expected to be obtained once eligibility requirements are met. Existing employees who held this position before July 1, 2021, are exempt from this requirement.
- Knowledge of Substance Use Disorder (SUD) treatment and/or ASAM criteria.
