Saint Augustine, St. Johns County, Florida

Hourly Rate: $18.23

Paid Bi-weekly.

MINIMUM QUALIFICATIONS:

  • Experience working in a 120-bed skilled Long-Term Care facility
  • Knowledge of Medicaid/Medicare eligibility or insurance processes.
  • Must have excellent communication and interpersonal skills.
  • Must be fluent in the English language: reading, writing, speaking.
  • Must have basic skills in the use of Microsoft Office Outlook and Word.
  • Must be willing to work evenings and weekends as needed.
  • Must be willing to provide management and coordinate case management, treatment, referred counseling and follow up services for residents.
  • Must be a Florida Licensed Clinical Social Worker
  • Successful completion of the employment screening process to include, but not limited to, a national background investigation and drug test.‎

PREFERRED QUALIFICATION:

  • Bachelor’s degree. 
  • Knowledge of Health Insurance Authorizations & Eligibility & knowledge of Medicare A.

PERFORMANCE STANDARDS AND EXPECTATIONS:

  • Works under the supervision of the Veteran’s Home Administrator
  • Develop and implement policies and procedures for social services.
  • Perform duties as backup to the Admission Coordinator when needed.
  • Provides management, coordination and oversight for the social services department.
  • Monitors participation of the veteran resident in programs/services designed to meet psychosocial, mental and emotional needs of the veteran.
  • Participates in Interdisciplinary Treatment team meetings to develop a care plan.
  • Provide knowledge of Medicaid benefits to families and residents.
  • Perform other duties as assigned.

This position is in the Other Personal Services (Temporary) system
