As an Operations Manager in Care Services, you’ll make a meaningful impact, helping to ensure patients receive the right care, at the right time, in the right place, with the support to succeed in your role.

Job Duties:

You'll provide direction and operational leadership to Care Management for assigned clinical areas. You'll lead daily care management operations including clinical practice, budget accountability, regulatory and compliance outcome accountability.

  • Requirements include: At least 3 years of supervisory/managerial experience and at least 3 years of clinical experience, Bachelor's Degree is required

  • Preferred experience: BSN or MSW with prior case management or care coordination experience

  • Typical day is 8:00 AM to 4:30 PM - and after-hour manager support is required

  • About the team/Clinical Make-up Our Geisinger Community Medical Center Care Coordination team is comprised of Registered Nurses, Social Workers, Case Workers and Care Management Assistants

  • Making Better Health Easy: Our team utilizes their critical thinking skills to produce positive outcomes for our patients. The care coordination team collaborates closely with providers and the inpatient units to deliver exceptional patient outcomes. Our team works to the top of their licenses/degrees and uses their training while advocating for the proper level of care for their patients all while maintaining a good work/ life balance.

Position Details:

You'll develop and deploy hardwired care transition strategies for patients across the care continuum, with a focus on engineering out transition failures, avoidable readmissions and avoidable emergency department utilization.

Responsible for optimizing patient and family outcomes through optimizing care management processes, workflows, communications, technology development and adoption. Develops and responds to clinical and financial data in care management program design, implementation and evaluation.

  • Participates effectively as a member of departmental and divisional and regional leadership teams.
  • Employs transformational leadership concepts in the development, implementation, and evaluation of a culture of shared decision making.
  • Facilitates continuing education and research opportunities for care management team, self and community to support growth.
  • Utilizes team building concepts to effectively empower team member by serving as a mentor, teacher and coach.
  • Contribute to the development of employee orientation, continuing education and competencies.
  • Maintains membership in a professional nursing or care management organization.
  • Enhances skills of team members such as decision-making, problem solving, conflict resolution, human resource management and communication.
  • Supports and promotes activities that are consistent with promoting a culture of innovation.
  • Facilitates development, maintenance and success of department teams.
  • Demonstrates accountability for continuously improving the department, division, and system by developing performance expectations and goals that focus on service, quality and cost.
  • Contributes to the design and development of programs in coordination with department leadership.

Education:

Bachelor's Degree-Related Field of Study (Required), Master's Degree-Social Work (Preferred)
