JOB SUMMARY

The Oncology Patient Navigator serves as a central point of contact for patients and families throughout the cancer care continuum, including diagnosis, treatment, survivorship, and supportive care. Working collaboratively with physicians, nurses, social workers, schedulers, and other interdisciplinary team members, the Patient Navigator coordinates services, facilitates access to care, removes barriers to treatment, and promotes continuity of care. The role supports patient-centered care, health equity, and improved patient experience through education, advocacy, resource coordination, and timely access to services.

EDUCATION

Preferred associate or bachelor’s degree in healthcare-related field or equivalent experience.

EXPERIENCE

Minimum two years’ healthcare experience; oncology, navigation, case management, or care coordination experience preferred.

This is a bargaining union position.
