Pay Range:

$30.94 - $60.74

Practice: Central Georgia Cancer Care

Location: 800 1st St #410, Macon, GA 31201

Schedule: Full Time, Monday-Friday 8a-5p

Previous oncology experience required. Candidates with an active OCN will recieve an additional $2.25/hr.

An oncology nurse navigator (ONN) is a professional RN with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. supporting organizational strategies across the cancer care continuum, necessitating the ONN to master knowledge, skills, and tasks that allow him or her to influence network systems and behaviors, community needs assessment, navigation program development, marketing, and outreach, understanding and use of ONN-specific outcomes metrics, and identification of gaps in and strategies to meet continuum-based needs. In addition, cancer survivorship planning is increasingly a component in the navigator role expectations

Key Performance Areas

KPA 1 – Collaborate with Healthcare providers

  • Coordinate the care of patients with a past, current, or potential diagnosis of cancer while adhering to NCCN guidelines.

  • Assist patients with cancer, families, and caregivers to overcome healthcare system barriers

  • Coordinate extension of services to include nutrition, home health, DME and hospice

  • Coordinate cancer survivorship planning

KPA 2 – Patient and family education

  • Provide education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer care continuum (CCC).

KPA 3 – Communication

  • Ensure effective communication and premier customer service with internal and external customers

  • Communicate relevant updates and provide quarterly patient services data to director

  • Collaborate with community navigators and assist with resources, education and planning

  • Participate on committees to meet requirements for accreditation

KPA 4 – Clinical Trial access

  • Collaborate with healthcare providers on potential trials

  • Participate in screening and consenting patients for clinical trial

  • Assist with startup of trials to include regimen requests and requirements for blinded studies as assigned

KPA 5 – Teamwork

  • One team approach to maintain in-depth understanding of program requirements.

  • Assess patient needs for resources and assist lay navigator

  • Participate in outreach programs

  • Coordinate Specialty Infusion Services with lay navigator and training with staff

  • Provide advanced care planning education and documentation with lay navigator

  • Collaborate with Liasion/OM to provide up to date marketing materials for providers, patients, and families

  • Performs other duties and projects as assigned.

Position Qualifications/Requirements

Education

Minimum of Associate degree in Nursing, Bachelor of Science in Nursing preferred.

Previous Experience

  • Minimum of five (5) years’ experience in Oncology nursing

  • Clinical trial experience preferred

Self-motivated and self-starter with ability work independently with limited supervision. Ability to work remotely effectively as required. Must be able to work effectively in a fast-paced, multi-site environment with demonstrated ability to juggle competing priorities and demands from a variety of stakeholders and sites.

Computer Skills

Proficiency in MS Office Word, Excel, Power Point, and Outlook required.

Certifications/Licenses

  • Registered Nurse required

  • OCN certification required

  • Valid Driver’s license

  • Current BLS

  • ONN certification preferred

  • Public Notary

  • Valid state Driver’s License for travel to satellite offices and offsite meetings. Compliance with the company Driver Safety Operations and Motor Vehicle Records Check Policy is required.
