On-Call Registered Nurse (RN)

Full-Time Hospice On-Call Nurse

Schedule:

  • 4p-12a- 5 shifts per week to total 40 hours/week.

We're seeking an experienced On-Call Registered Nurse to provide after-hours clinical support to hospice patients and families. What You'll Do

Respond When Patients Need You Most

  • Provide after-hours nursing care and symptom management for hospice patients
  • Respond to urgent patient and family concerns during evenings, nights, weekends, and holidays
  • Make home visits as needed to assess patients, manage symptoms, and support families in crisis
  • Provide compassionate support at the time of a patient's death and assist families through the immediate bereavement process

Deliver Expert Hospice Care

  • Assess, plan, implement, and document personalized hospice care
  • Collaborate with physicians and the interdisciplinary team to ensure patients receive the right level of care
  • Monitor and coordinate medications, medical equipment, and supplies
  • Educate patients and families regarding symptom management, disease progression, and end-of-life care

Collaborate and Lead

  • Work closely with triage nurses, managers-on-call, and interdisciplinary team members
  • Support orientation and mentoring of new team members
  • Contribute to patient-centered care planning and clinical excellence across the organization

Required Qualifications

  • Current Registered Nurse license in North Carolina
  • Minimum three years of recent clinical nursing experience
  • Current CPR certification
  • Strong assessment, critical thinking, and clinical management skills
  • Valid North Carolina driver's license
  • Reliable transportation and automobile insurance
  • Ability to travel throughout the service area and provide care in homes and facilities
  • Competent computer and documentation skills
  • Ability to lift and/or carry up to 50 pounds
  • Availability to work scheduled holidays, weekends, and overnight shifts
  • Ability to attend a minimum of four team meetings annually

Preferred Qualifications

  • BSN
  • Hospice nursing experience
  • Home health experience
  • Skilled nursing or long-term care experience
  • Certified Hospice and Palliative Nurse (CHPN) credential
  • Knowledge of Medicare regulations, hospice reimbursement, accreditation standards, and regulatory compliance
