ESSE NTI AL JOB FUNCTIO NS/RESPO NSIBILIT IES
Patient Care
1. Co mpletes initial, co mprehensive and ongoing assess ments of patient and
fa mily to de termine hospice needs.
2. P rovides a co mple te physical assess ment and history of current
and p revious illness (es )
3. P rovides p rofessional nursing ca re by utilizing all ele ments of nu rsing
p roce ss.
A. Cont ributes to the plan of ca re
B. Pa rticipating in revising the plan of care as necessa ry
4. Uses health assess ment data to dete rmine nu rsing diagno sis.
6. Initia tes app rop riate p reventive and rehabilitative nu rsing p rocedu res.
7. Ad ministe rs medica tions and t rea tmen ts as p re scribed by the physician
in the physician’s plan of ca re.
8. Counsels the patient and family in meeting nu rsing and related needs.
9. Adhe ring to the o rganization’s docu mentation and ca re p rocedu res and
standa rds of pe rsonal and p rofessional conduct.
10. P rovides health ca re inst ructions to the pa tient as app rop ria te pe r
asse ssment and plan.
11. Assists the patient and/or caregiver/family with the activities of daily
living, provision and education, and facilitates the patient's efforts toward
maintaining independence for as long as possible and optional comfort
of care.
12. RN will act as Case Manager until a Case Manager is assigned by Clinical
Supervisor or designee and assumes responsibility to coordinate patient
care for assigned patient.

and recognizes and utilizes oppo rtunities fo r health counseling with patien ts
and fa milies/ca regive rs. Wo rks in conce rt wi th the inte rdisciplina ry g roup.
5. P rovides and maintains a safe environ ment fo r the patient.
6. Assists the patient and fa milyca regive r and other team me mbe rs in
providing continuit y of ca re.
7. Wo rks in coope ration with the fa mily/ ca regive r and hospice In te rdis ciplina ry

1. Is responsible for all on -call duties as defined by the on -call polic y

POS ITIO N QUALIFICATIO NS
1. Registe red nu rse wi th cu rrent licensu re to p rac tice p ro fessional nu rsing in the
s ta te.
2. Graduate of National League fo r Nursing acc redited school of nu rsing.
3. Maintains a current CPR certifica tion.
4. Must be a licensed d river with an auto mobile that is insu red in accordance
with state or o rganization requi re ments and is in good wo rking o rder.
5. Mini mum of two years' experience, at least one of which is in the area o f
publi c health, ho me ca re, or hospice nu rsing is p re fe rred.
6. Self-directed and able to wo rk wi th mini mal supervi sion.
7. Manage ment expe rience not required. Re spon sible for supervi sing ho me
health aide if weekend vi si ts are assigned for aide.
8. De monst ra tes excellent obse rvation , proble m solving, ve rbal and w ritten
co mmunications; nu rsing skills per co mpeten cy che cklist.
9. Shows ability to o rganize and p rio ritize wo rkload independently.
