Brooklyn, Queens, and Jamaica Community Living Programs provide services to adults with a severe mental illness, a DSM diagnosis is required for a client to be eligible for services and need of housing.
STATEMENT OF THE JOB
Under the direction of the Residential Supervisor, the OMH- Resident Wellness Coordinator is responsible for providing rehabilitation and recovery services to residents of Residential Programs funded by the New York State Office of Mental Health (NYSOMH). The OMH- Resident Wellness Coordinator provides counseling, advocacy and case management services to residents residing in NYSOMH programs. The OMH-Resident Wellness Coordinator ensures that all residents are treated with respect and dignity. The OMH-Resident Wellness Coordinator implements best practices in accordance with the Part 595 Regulations of the New York State Office of Mental Health as well guidelines implemented by the Agency.
The OMH-Resident Wellness Coordinator provides Service Coordination as the agency has embraced the concept of a fully integrated system of care for all consumers of service. This requires that the staff assess the client's needs holistically and see it as their responsibility to connect the client with services needed--internal or external to the agency, regardless of the door to which the consumer enters the system.
