OhioRISE, or Resilience through Integrated Systems and Excellence, is Ohio’s first highly integrated care program for youth with complex behavioral health and multi-system needs. Care Management Entities (CMEs)  are vital to the success of the OhioRISE model and will each serve a separate catchment area – part of a county or multiple counties that make up their geographic footprint.

QualificationsJob Title: Associate Director of Community Relations & Expansion
Reports To: SVP of Care Management

Summary:
Utilizing high fidelity wraparound services, OhioRISE child and family teams provide moderate and intensive care coordination for Ohio’s most behaviorally complex children. This position oversees the development of quality relationships with community agencies and the public resulting in the expansion of formal partnerships to serve OhioRISE members.
Essential Tasks, Duties, and Responsibilities:
- Facilitate monthly meetings with system partners to assess needs and to evaluate the effectiveness of OhioRISE care coordination for shared consumers;
- Attend all scheduled meetings by system partners intended to enhance the local system of care
- Provide education and training sessions to promote awareness of and access to OhioRISE Care Coordination for system partners, schools, non-profit organizations, etc.
- Represent TVN and the TVN OhioRISE CME at local, state and federal conferences
- Partner with EVP of Communications to ensure promotion of OhioRISE on TVN social media platforms and website content.  Represent TVN on the Aetna Communications committee
- Collect, analyze and share data related to member referrals, linkages and outcomes
- Oversee contract, hiring, onboarding, and training for subdelegate expansion
- Other duties as assigned
Knowledge, Skills, and Abilities:
- LSW or PC licensure required; Independent licensure preferred
- Must complete the high-fidelity wraparound training program provided by the Center of Excellence, an entity recognized by ODM.
- A minimum of 2 years of experience in community liaison work
- Experience in professional networking, marketing or system collaboration preferred
- Familiarity with Franklin County non-profit and government systems
- Be culturally competent or responsive with training and experience necessary to manage complex cases.
- Ability to use a computer; proficiency in Word and Electronic Health Record (EHR). 
- Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance. 
- Willingness to travel for various reasons, mainly during the day but occasionally overnight.  Ability to visit clients’ homes (may or may not be handicapped accessible).
- Excellent verbal and written communication skills; strong teamwork and organization/time management skills.
- Maintain a HIPPA compliant home-based office space for use when remote work is pre-approved by direct supervisor
Physical Demands
- Occasionally move about inside the office to access file cabinets, office machinery, etc.
- Constantly operates a computer and other office productivity machinery, such as a copy machine, computer printer, etc.
- Ability to effectively communicate (orally and written) with all levels of personnel, internal and external to the company
- Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
- Must be able to lift 20 pounds independently
