Job Description

Under the general supervision of the Specialty Practice Manager provides professional nursing care to adult patients in our ambulatory cardiology outpatient office. Nurse within onsite clinic will support remote cardiologists by performing cardiac screening and diagnostic testing support, triage, patient education and coordination of care in community. Act as a clinical resource to providers, clinic staff, and patients while ensuring high-quality, safe and efficient outpatient cardiology services that meet regulatory and hospital standards.

Direct Clinical Care:

• Triage telephone and in person cardiology concerns; assist with medication refills, prioritize and escalate urgent issues to providers or emergency services per protocol.

• Perform pre-visit assessments and history review relevant to cardiology care (Cardiac risk factors, current medications, allergies, symptoms and vitals)

• Accurately documents patient's chief complaint, medical history, and vital signs and completes pertinent forms within the Electronic Medical Record and in accordance with the Best Practice Workflows

• Prepare patients for and assist with outpatient cardiac diagnostic testing, including electrocardiograms (12-lead ECG) and ambulatory monitoring (Holter/event monitors).

• Provide patient and family education regarding cardiac diagnoses, medications (anticoagulation, heart failure regimens, antianginals), lifestyle modification, post-procedure care and follow up instructions.

• Coordinate referrals, diagnostic testing, consultations and follow – up appointments with specialists, community resources and hospital departments.

• Assist providers with clinic procedures as needed (wound-care related to device sites when applicable, suture removal and telemetry hookup and removal, and device clinic)

• Document all patient care, triage notes and clinical activity accurately and timely in the electronic Health record in accordance with EMR’s Best Practice

Clinical Support:

• Maintain competency in ambulatory cardiology policies and procedures; participate in quality improvement initiatives, audits and infection control practices

• Ensure compliance with HIPAA< CMS and hospital policies

• Works with Practice Manager and Director to improve clinical workflows within the practice

• Support telehealth visits by preparing and education patients of the process, perform pre-visit assessments and trouble shoot basic telemedicine connectivity.

• Introduce patient to cardiologist on the Tele medicine computer cart

• Participate in community outreach/education events as needed to support cardiac health initiatives

• Acts as a clinical liaison between patients, remote Cardiologists, primary care providers, specialty services and vendors (remote monitoring/device companies)

• Maintain inventory of clinic supplies and cardiac testing equipment; ensure proper function and coordinate vendor maintenance as needed

Supply, Equipment and Medication Coordination:

• Supervises the maintenance of an inventory of supplies and equipment as required to perform clinical services

• Ensures that emergency equipment is checked and functional

• Ensures hazardous materials are stored, handled, and disposed of in accordance with Hospital policies

• Works in collaboration with the Pharmacy Department regarding patients who are in need/eligible for sample medications, adhering to established process of communication and procurement at Huggins pharmacy

• Orders medical supplies and medications needed and keeps exam rooms stocked

·Minimal physical effort required with occasional light lifting to a maximum of 25 pounds; stooping; and pick, pinch, type, or otherwise work primarily with fingers

·Ability to communicate effectively with co-workers and convey information in a clear, concise and accurate manner

·Familiarity with computer and other business machines

Qualifications:

Completion of RN program, MSN preferred

Current NH RN License

BLS Required

Minimum of 2 years nursing experience; experience in ambulatory care, cardiology, emergency or telemetry preferred.

Strong clinical assessment, communication and organization skills

Ability to work independently and collaboratively with other departments

Ability to seamlessly work with a purely remote cardiologist
