Job Description

Job Location 125A Amory St - Jamaica Plain, MA 02119 Salary Range $43.00 - $58.00 Hourly

Primary Function

  • Provide occupational therapy interventions to PACE participants through assessment, re-assessment, treatment planning, and implementation with the goal of maximizing functional skills and independence.
  • Supervise and coordinate home care services for PACE participants.

Pay Range: The expected base pay for the position is $43-$58 per hour and may be increased based on other factors, such as language, certifications, etc.

Duties & Responsibilities

  • Conduct initial Occupational Therapy and Home Care evaluations and ongoing reassessments for PACE participants to promote independence and community living.
  • Perform home evaluations and safety screens semi-annually to optimize ADLs and ensure a safe environment.
  • Develop and implement individualized care plans in collaboration with the Interdisciplinary Team (IDT).
  • Educate and supervise participants, caregivers, and staff to ensure adherence to rehabilitation principles.
  • Collaborate with PCW Supervisor for introductions and supervision of personal care workers.
  • Participate in discharge planning from hospitals and nursing homes.
  • Assist with clinical competency training for Health Aides as needed.
  • Maintain accurate documentation in the EMR system.
  • Recommend and order durable medical equipment and adaptive devices for participants and PACE centers.
  • Participate in team meetings, quality improvement activities, and program development projects.
  • Perform other duties as assigned by the Manager.



Qualifications



  • Experience:

    • 2–3 years working with frail elderly or chronically ill individuals preferred.
    • Comfortable working with a multicultural population.
    • Strong interest in geriatric care; flexibility, creativity, and organizational skills.
  • Licenses & Certifications:

    • Valid Massachusetts Occupational Therapy license.
    • CPR certification required.
    • Valid driver’s license and access to a car.
    • Bilingual skills a plus.
