Provide pediatric occupational therapy services to patients who have impairments, functional limitations, physical/developmental disabilities or changes in physical function and health status resulting from injury, disease or other causes. Actively collaborates with the health care team to meet patient care, outcomes and system improvement objectives.

Make a Difference for Medically Complex Children at Home

Vanderbilt Health is seeking a compassionate Occupational Therapist to join our Pediatric Home Health team serving children throughout Davidson and Williamson Counties.

This role is ideal for an OT with a passion for pediatric care, infant development, neurodevelopmental intervention, and family-centered practice. You'll work primarily with children discharged from Monroe Carell Jr. Children's Hospital, with a variety of ages and diagnoses including patients from the NICU, Cardiac Unit, general floors and PICU, helping them achieve developmental milestones and maximize independence within their home environment.

As part of Vanderbilt Home Care Services, you'll deliver evidence-based occupational therapy while partnering closely with families and an interdisciplinary team dedicated to improving outcomes for medically complex children.

The Children You'll Serve

The majority of referrals originate from Monroe Carell Jr. Children's Hospital and include children with:

• Prematurity
• Congenital and acquired cardiac conditions
• Neurological diagnoses
• Developmental delays
• Feeding and sensory challenges
• Orthopedic conditions
• Genetic disorders
• Medically fragile and technology-dependent needs

This position offers a unique opportunity to support children and families during critical developmental periods while helping bridge the transition from hospital to home.

What You'll Do

• Perform occupational therapy evaluations in the home setting
• Develop individualized treatment plans and goals
• Provide evidence-based interventions focused on developmental, sensory, motor, feeding, and functional outcomes
• Educate and coach parents and caregivers on home programming and therapeutic strategies
• Assess environmental factors and recommend adaptive equipment as appropriate
• Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and case managers
• Participate in interdisciplinary care planning
• Complete timely clinical documentation within the electronic medical record

Qualifications

Required

• Graduate of an accredited Occupational Therapy program
• Current Tennessee Occupational Therapist license
• CPR certification
• Valid driver's license and reliable transportation
• Strong communication, organization, and documentation skills

KEY RESPONSIBILITIES

PLANNING AND MANAGING CARE

• Formulate functional treatment plan that is goal focused, individualized, safe and age-specific to address deficits and patient/caregiver areas of concern.
• Evaluate patient/caregiver responses and modify plan of care according to VHCS and VUMC standards.
• Administer and score standardized and criterion referenced tests.
• Interpret the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team.
• Evaluate and incorporate new research findings into the assessment and treatment of patients.
• Assist with establishing or modifying existing "Standards of Practice" for specific patient groups.
• Complete documentation according to agency policy.
• Meet productivity standards.

• Participate in identifying cost effective measures for clinical care.
• Manage clinical assignment in an organized, efficient manner using human and other resources.

• Assure coordination of care for all patients including multi-disciplinary coordination as a case manager.
• Identify and recommend for referral to other services as indicated.
