Occupational Therapist (OTR/L) – Part Time

Cedarburg, WI

Concept Rehab & Engage Consulting is looking for a compassionate and dedicated Occupational Therapist (OTR/L) to join our team Part Time in Cedarburg, WI.

This is a great opportunity for an OT looking for flexibility, a supportive therapy team, and meaningful work with patients in a skilled nursing and rehabilitation setting.

What You’ll Do

• Evaluate patients and develop individualized occupational therapy plans of care
• Provide skilled OT treatments designed to improve function, independence, safety, and quality of life
• Establish measurable, functional goals based on each patient's needs
• Monitor progress and modify treatment plans as appropriate
• Supervise and collaborate with Occupational Therapy Assistants
• Communicate effectively with patients, families, nursing staff, physicians, and other members of the interdisciplinary team
• Participate in care plan and interdisciplinary team meetings
• Complete timely and accurate evaluations, progress notes, discharge summaries, and other required documentation
• Provide education to patients, families, caregivers, and facility staff
• Recommend adaptive equipment and strategies to improve patient independence

What We’re Looking For

• Graduate of an accredited Occupational Therapy program
• Current Wisconsin Occupational Therapist license
• NBCOT certification
• Strong clinical, communication, and organizational skills
• Compassionate and patient-centered approach
• Skilled nursing, long-term care, or rehabilitation experience is a plus
