Victorian Post Acute is a beautiful 90-bed, rehab-focused skilled nursing facility serving both short-term rehabilitation and long-term care residents. We are seeking a compassionate and motivated PRN Occupational Therapist (OTR) to join our dedicated in-house therapy team.

Key Responsibilities
Evaluate residents and develop individualized occupational therapy treatment plans in collaboration with the referring physician and interdisciplinary team.
Provide skilled occupational therapy services according to the established plan of care.
Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with California practice guidelines.
Train and support Restorative Nursing Assistants as appropriate.
Participate in interdisciplinary care conferences, rehabilitation meetings, and discharge planning.
Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
Educate residents, families, and caregivers on home exercise programs, adaptive equipment, and safety recommendations.
Recommend and coordinate appropriate durable medical equipment (DME) to maximize resident independence and safety.
Report equipment concerns promptly to maintain a safe therapy environment.
Qualifications
Current California Occupational Therapist (OTR) license in good standing.
Strong communication and collaboration skills.
