Pleasant Hill Post Acute is seeking a compassionate, motivated, and dedicated Occupational Therapistto join our rehabilitation team. We offer Full-Time, Part-Time, and PRN opportunitiesfor OTs who are passionate about helping patients regain independence and achieve their highest level of function.
Our rehabilitation program emphasizes quality patient care, teamwork, communication, and collaborationacross nursing, therapy, and other departments.
What You'll Do
Evaluate patients and develop individualized treatment plans based on their functional needs and goals.
Provide skilled occupational therapy services to help patients improve independencewith activities of daily living.
Help patients maximize functional abilities and improve their overall quality of life.
Monitor progress and adjust treatmentplans as appropriate.
Collaborate with physicians, nursing staff, physical therapists, speech therapists, and other members of the interdisciplinary team.
Communicate effectively with patients, families, and caregivers regarding treatment goals and progress.
Complete accurate, timely, and compliant clinical documentation.
Providecompassionate, patient-centered care while maintaining professional standards.
Qualifications
Current California Occupational Therapist (OT) licensein good standing.
Strong clinical, communication, and organizational skills.
Ability to work effectively as part of an interdisciplinary team.
Skillednursing or post-acute rehabilitation experience preferred.
New graduates are welcome to apply.
