Your Role

• Evaluate residents and develop individualized treatment plans based on medical diagnoses
• Implement therapy programs to improve functional independence and daily living skills
• Monitor and document resident progress through daily notes, progress reports, and re-certifications
• Maintain accurate and compliant documentation of care services
• Supervise and provide clinical guidance to Occupational Therapy Assistants (OTAs)
• Participate in care plan meetings and interdisciplinary collaboration
• Assist with departmental projects, reporting, and data tracking as directed
• Ensure all care is delivered in compliance with state, federal, and facility guidelines

Qualifications

• Graduate of an accredited Occupational Therapy program
• Active New York State OT License required
• Strong clinical, documentation, and communication skills
• Experience in skilled nursing or rehab setting preferred
• Team-oriented with a commitment to high-quality patient care
