The Occupational Therapist provides skilled occupational therapy services in accordance with the patient’s plan of care, including initial and ongoing assessments, evaluation of patient progress, and individualized treatment. This role ensures accurate documentation and delivery of services in accordance with physician orders and accepted standards of occupational therapy practice, under the direction and supervision of the Home Health Director. The Occupational Therapist models and promotes our core values of respect, passion, and professionalism and hold others accountable for doing the same.

What You’ll Do In This Role

• Provide skilled occupational therapy services to improve or minimize physical disabilities and help patients achieve their highest level of independence and productive living within their capabilities.
• Complete initial and ongoing assessments, develop individualized plans of care and goals, and modify treatment and maintenance programs based on patient progress and needs.
• Provide prescribed occupational therapy and skilled procedures in accordance with physician orders, accepted standards of practice, and the patient’s plan of care.
• Complete required clinical documentation, including visit notes, progress reports, reassessments every 30 days, reevaluations, discharge summaries, and documentation required for physician recertification every 60 days when continued treatment is needed.
• Communicate with physicians regarding changes in patient condition, treatment recommendations, functional status, and progress; assist physicians in evaluating the patient’s level of function.
• Educate patients and family members on home programs, fine motor exercises, treatment techniques, and strategies to support continued progress and independence.
• Collaborate with physicians, family members, and other home care personnel in developing, coordinating, and updating the patient’s plan of care.
• Direct and supervise personnel as required and provide guidance to agency staff regarding patient care.
• Participate in agency inservice programs and provide inservice education to staff as assigned.
• Participate in Quality Assurance and Performance Improvement (QAPI) activities as assigned.
• Perform other duties as assigned.

Physical Requirements

• Ability to travel to patient homes and community locations
• Ability to lift, push, pull, or move a minimum of 50 pounds
• Capable of performing physical assessments including standing, bending, and kneeling
• Must be able to perform cardiopulmonary resuscitation and relief of an obstructed airway
• Climb at least 2 flights of stairs
• Able to stand and/or walk for a period of at least 4 hours

The Experience You’ll Bring

Required

• Current Occupational Therapist license in the state of Virginia.
• Graduate of an accredited occupational therapy program.
• 2 years of experience with a geriatric/senior community.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and ability to travel to patient homes.
• Experience with Microsoft Office and Google Suite
• Flu-Vaccinated

Preferred

• Previous home health experience.
• Experience with Medicare home health documentation and regulatory requirements.
• Experience working with older adults, patients with functional decline, fall risk, neurological conditions, orthopedic conditions, or chronic illnesses.

• Compensation Range: $100K - $125K annually (based on relevant experience)
