As an OT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program!
What is Take Your Therapist Home?
In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is a one-of-a-kind program where you as the clinician work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes!
You will be primarily in the home health setting with dedicated time and caseload support for the SNF. You will have a localized coverage area in the surrounding area of the SNF. No more ever-expanding home care coverage areas. Your schedule is based on a traditional work week.

Our perspective is unique in providing continuity of care across the health care continuum in the Skilled Nursing, Home Health, and Outpatient settings.

Facility Location: Troy- 925 West South Blvd Troy, MI 48085
Schedule: Monday - Friday, Full Time
Required Qualifications:
- Successful completion of an accredited Occupational Therapy program.
- Licensure as required by the State of Michigan.
Skills and Abilities:
- Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
- Initiative and flexibility in the work environment.
- Ability to react effectively to the needs of patients and their families.
- Physical Ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
