Job Description

Liquid Personnel is recruiting a General Occupational Therapist to join their Adult Social Care OT service for its client. This is a broad community-based role, suitable for an Occupational Therapist who is confident in undertaking functional assessments, recommending equipment and adaptations, and working independently across the borough.
You will support adults with a wide range of physical, cognitive, and social needs, enabling safer and more independent living within their homes and communities.

Working Pattern: Hybrid / community-based (as required by the service)

What will your responsibilities be?

  • Complete holistic functional assessments in homes and community settings.
  • Recommend and arrange equipment and minor adaptations to support safety and independence.
  • Apply strengths‑based, person‑centred approaches in all assessments.
  • Develop and review moving & handling plans, ensuring safe techniques and proportionate recommendations.
  • Work closely with social workers, housing teams, health colleagues, families, and carers.
  • Maintain clear, accurate, and timely case records in line with service standards.
  • Contribute to prevention‑focused practice, early intervention, and effective risk management.
  • Support timely discharge planning and community reablement pathways where required.

Qualifications & Experience:

To be successful in this role you must have,

  • Degree / Diploma in Occupational Therapy.
  • Experience working in adult social care, community OT, reablement, or similar settings.
  • Confident with equipment provision, risk assessments, and clinical decision‑making.
  • Strong communication and problem‑solving skills.
  • Ability to manage an independent caseload in a busy environment.
