Serving a variety of patients across Southern Colorado, including pediatric patients and those with developmental disabilities, Sangre de Cristo Community Care is known for delivering exceptional high-quality compassionate care with a strong team of local health care professionals, including skilled nursing, physical therapy and occupational therapy, providing a seamless opportunity for high-quality hospice, palliative care, and home health services to residents in southern Colorado.
The primary role of the Occupational Therapist is to direct all aspects of quality patient care, including evaluation, treatment, treatment planning, goal setting, family education and documentation according to organization’s guidelines, professional standards and community needs. This position will also serve as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers while participating in quality assurance and customer service activities.

Qualifications:
Graduate of an approved curriculum in Occupational Therapy, with two years of clinical experience preferred. Can practice unencumbered, is in good standing with all regulatory agencies and licensing boards, has working knowledge of Medicare and other payer sources, full knowledge of residents’ rights, exude professionalism in presentation, able to work on multitasks at the same time. In addition, qualified candidates must have proven success in providing extraordinary care, working as a proactive and contributing part of a dynamic team. Customer service expertise is crucial to ensure patient care is performed with kindness, competence, and compassion.
