Job Description

MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:

Under the supervision of their supervising or collaborating Physician the Family Nurse Practitioner/Physician Assistant will:

  • Perform comprehensive initial medical assessment and ongoing check-ups for all prenatal patients in accordance with protocols.
  • Perform women’s health care examinations and provide basic GYN care, contraceptive, and STD services according to clinical protocols.
  • Accurately complete all required documentation according to protocols.
  • Order lab work, perform other procedures and perform required follow-ups in accordance with protocols.
  • Identify high-risk patients, and refer to OBGYN physician or to outside sources in accordance with clinical protocols.
  • Perform timely documentation/action within Electronic Health Record (EHR) including but not limited to, diagnosis, treatment plan, orders, medications, etc.
  • Take timely action on pending EHR PAQ items.
  • Issue timely responses to E-Consult specialist reviewers.
  • Adhere to Quality Improvement guidelines and participates in Quality Improvement programs for chronic diseases.
  • Maintain and update, as needed, all educational materials and patient instruction sheets.
  • Participate in monthly Provider Meetings as required.
  • Participate in team meetings as required.
  • Serve as a preceptor instructor to Nurse Practitioner, Physician Assistant, or medical students as assigned.
  • Attend conferences and seminars when requested and will comply with continuing education requirements.
  • Participate in provider recruitment activities.
  • Participate in activities that foster the development of clinical support team members.
  • Work towards increasing effectiveness of the OBGYN program in conjunction with the Chief Medical Officer and OBGYN Associate Medical Director.
  • Ability to multi-task and prioritize daily workload
  • Other assigned duties as directed.

POSITION REQUIREMENTS AND QUALIFICATIONS:

  • Satisfactory completion of an accredited Advanced Practice Provider training program.
  • Completion of accredited Nurse Practitioner or Physician Assistant training program in obstetrics.
  • Current license in the State of California as a Nurse Practitioner/Physician Assistant.
  • Current DEA certification.
  • Current Board Certification from accredited organization for the certification of Advanced Practice Providers.
  • Current CPR certifications.
  • Must relate well to all cultural, ethnic, and socio-economic groups within the East Valley service area.
  • Able to work with minimal supervision.

DOE: $138,000 - $153,739 Annually

Who We Are

We are a Federally Qualified Health Center (FQHC) that provides family medicine, pediatrics, women’s health, behavioral health, dental and vision services to individuals across the East San Gabriel Valley and Pomona Communities. We practice patient-centered care and strive to serve each patient with the care that meets their individual needs.

Who We Serve

Our service area is a large, diverse, complex and rapidly changing geographic area that is home to over 700,000 people and faces significant challenges in chronic disease, including diabetes, hypertension and asthma. The majority of our patients are the working poor, and 62% are uninsured. East Valley holds public and private contracts with many funding sources, which allow us to provide medical services at very low cost to qualifying individuals. East Valley is also affiliated with two local IPAs and holds managed care contracts with most health plans operating in California. Fees for our services are on a sliding scale, but no one is turned away because of inability to pay. East Valley accepts all public health insurance, such as Medi-Cal (also known as Medicaid), Medicare, and public programs such as My Health L.A. as well as private health insurance.

Most importantly, if you or your family are not covered by any health insurance plan, our certified enrollment counselors will help you apply for Covered CA, Medi-Cal, Medicare so you can be assured your that you are accessing the best coverage options for and your family. If you don’t qualify for health insurance we can assist you with public programs, such as My Health L.A. (MHLA) in accordance to their eligibility requirements.

East Valley is a medical home for people who cannot be seen or cared for anywhere else.
