Job Description

The Nutritional Coordinator is responsible for nutritional assessment of the outreach program participants. The Coordinator performs monitoring of patients diets and programs and confirms diet and patterns. The Coordinator will oversee nutritionists as required and evaluate the treatment programs according to established nutritional guidelines.

Responsibilities

- Supervises Nutritional component of program, assists in evaluating nutritional guidelines. Develops/revises assessment forms and other tools as needed. Develops curriculum for nutritional workshops and outreach presentations.

- Ongoing documentation of encounters in medical records. Documents short term and long term nutritional plans for patient population.

- Provision and documentation of comprehensive level clinical nutrition care and education to the patients that receive services the program sites. Counsels patients and/or family members on proper diet guidelines/healthy eating habits.

- Assesses and monitors patients’ nutritional status to determine appropriate nutrition care plan in accordance with medical diagnosis (I.e.: diabetes, high blood pressure, etc.). Reviews lab results with patients and/or family and revises nutritional plans as needed.

- Develops logs to track all encounters including individual, group and outreach. Develops systems to ensure that these logs are entered into program specific database. Assists with evaluation of quality assurance indicators.

- Functions as member of the multi-disciplinary health care team to provide nutrition education and care to the adolescents who receive services at any of the three program sites. Attends weekly case conferences and multidisciplinary meetings, consults with medical staff on appropriate nutritional intervention to maintain and improve patient nutritional status or prevent further deterioration of health.

- Confirms diet through nutritional assessment, evaluates diet patterns, food allergies, and resources. Outlines daily intake of calories, nutrients, fluids and supplements as needed.

- Develops and facilitates nutritional workshops, prepares agenda, documents attendance on attendance log and participation on progress note. Conducts in-service/presentations to health care professionals, hospital employees and the community.

- Performs outreach to outside agencies and community resources for assistance to coordinate care of the adolescent patients as needed.

- Exhibits knowledge of patient’s developmental needs and demonstrates skill necessary to deliver nutrition care to that population. Develops and maintains an effective rapport with patients to determine individual’s needs and the most effective methods of meeting those needs.

- Supervise CHW assigned to clinical practice.

Qualifications

- 2-4 years supervisory experience in Nutrition and Counseling

- Certified Diabetes Care and Education Specialist (CDCES)

- 1 year experience in Clinical Nutrition

- Registered Dietitian

- ADA Registration as Dietitian

- Baccalaureate in Food and Nutrition or Dietetics (Required)

- Basic Computer knowledge (Required)
