Albany Family Medicine (AFM), part of Community Care Physicians (CCP) — the Capital Region’s first and largest independent multispecialty medical group — is seeking a Nurse Practitioner or Physician Assistant to join our dedicated primary care team.
AFM is a cornerstone of the Albany community and is proud to serve as a teaching practice with an established Family Medicine Residency Program. Our practice blends compassionate, relationship-based care with a strong commitment to education, collaboration, and continuous learning. We care for patients across the lifespan and value the important role Advanced Practitioners play in both patient care and the academic mission of the practice.
As an Advanced Practitioner at Albany Family Medicine, you will provide comprehensive family medicine care in a supportive, team-based environment that includes attending physicians, residents, nurses, and clinical support staff. You will practice to the full extent of your license, supported by CCP’s extensive specialty network and resources.
This position offers no after-hours call, allowing you to maintain a healthy work–life balance while focusing on delivering high-quality, patient-centered care.
