The Hospice Supervisor oversees staff and is responsible to assure that staff follow policy/ and procedures and all state, local and federal rules and regulations. The supervisor is responsible to assure all accreditation standards (ACHC) are followed. Oversees Hospice staff and in coordination of hospice care. Nursing supervisors demonstrate competency and skills to ensure that care is provided to patients served in a professional and clinically competent manner.

  • Coordinates and supervises aspects of the provision of care for hospice patients under the agencies care.
  • Acts as a liaison between medical leadership and Hospice staff in coordinating Hospice care.
  • Under the direction of Patient Care Manager serves as an operational and clinical resource in decision making and other issues affecting patient care, psychological and emotional care and spiritual aspects of care for all Hospice patients.
  • Facilitates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
  • Provides clinical supervision of the nursing team in home health and oversees caseload and scheduling under the direction of the Patient Care Manager.
  • As delegated by the Patient Care Manger monitors patient clinical records to assure all requirements are met.
  • Provides feedback to the Patient Care Manager regarding employee competency and performance for employees supervised.
  • Provides individual counseling and supervision as needed to hospice team members.
  • Woks as a member of the interdisciplinary hospice team to assure that desired outcomes and quality standards are met.
  • Keeps Patient Care Manger appraised of department issues/concerns.
  • Participates interviews of new applicants as requested by Patient Care Manger.
  • Contributes to staff orientation and annual appraisals for staff as assigned by Patient Care Manager.
  • As requested by Patient Care Manager, may conduct a direct in-home visit and evaluation for staff.
  • Understands COP’s and ACHC accreditation standards for hospice.
  • Provides direct nursing services to patients in their place of residence on an as-needed basis.
  • Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.
  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.
  • Demonstrates/models the Network's Service Excellence Standards of Performance in interactions with all customers (internal and external).
  • Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.
  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
  • Complies with Network and departmental policies regarding attendance and dress code.

PHYSICAL AND SENSORY REQUIREMENTS:

  • Ability to lift 20-50 pounds.
  • Constantly standing and walking
  • Frequent Reaching Above the shoulder, twisting, bending/stooping, repetitive movements of elbows/fingers, repetitive movements of wrists, kneeling/climbing /squatting
  • Visual and auditory acuity are required to provide comprehensive care

QUALIFICATIONS

EDUCATION:

Registered nurse (BSN preferred) with current license to practice in the State of Pennsylvania

TRAINING AND EXPERIENCE:

Prior Supervisor experience preferred. Hospice experience preferred.
