JOB DUTIES AND RESPONSIBILITIES:
This an independent professional public health nursing position in the Bithlo Neighborhood Center for Families. The incumbent provides professional nursing to identify qualify participants in the programs such as: schools, homes, clinic, and community settings. Provides nursing support as needed at outreach/awareness and incident response events within the community to include mass immunization, emergency response activities, and community health engagements. Nursing support may be required outside of normal business hours to accommodate community needs during evening and weekends hours.
The Senior Community Health Nurse will provide case management services in any of the NCF locations (Taft, Union Park, Bithlo/Christmas, Englewood and Pine Hills). This case management includes identification of needs/goals; assessments; counseling (including education), ensuring participant can access medical care and has a medical home: referral and follow up of client/family needs, concerns goals and priorities in a holistic manner.
The nurse provides basic health screenings to the opt-in students, immunization case management, health education, fluoride treatment program, family counseling and/or management on student’s medical conditions. Services are provided through health management by individual contact, and conferences with the teacher, students, and parents (for students that opt in health services). Referral services request are received from school staff, these referrals are based on health problems, the action for services thru a referral process is home or school visitation to assess the concern, after assessment nurses refer and provide support to the families.
Provide immunization to the children in their community at the season of highest demand of this service (back to school season), this vaccination take place at the NCF location. The NCF nurse provides vaccines in the community following the CDC schedule vaccines guidelines and recommendations, the nurse is also responsible for vaccine storage and handling, and accountability for their assigned NCF sites to include the amount of the vaccine ordered, shipped, received, used and on hand; monitoring data logger temperatures, uploading data into Florida Shots and daily review of physical inventory, these duties include the accountability for the waste of vaccines.
The nurse will focus on health issues in the assigned schools setting for the students that opt-in for screening services ( vision, hearing, growth and development, scoliosis). Case management of the referrals for abnormal screenings. Makes home visits when appropriate. She will provide health education in the form of lectures, audiovisuals aids, individual contact and conferences with teachers, students, and parents to promote school and community health, which impacts on the students’ ability to grow and develop.
In the community services are provided to the Senior Citizen thru health screenings, health education and health counseling towards their chronic diseases such as diabetes and hypertension, home visit services are available upon to referral from Citizen Senior partner The nurse will assist and participate in the coordination of community activities related to health, such as: health fairs, health screenings, health education programs, wellness promotion, youth development and immunizations.
Review NCF quarterly reports with the other nurse to help ensure accuracy and completeness. Provide support staff. Train and orient new nurses to the NCF program. Review EAR entry to ensure timely input. Attend NCF meetings, CCC Conference and OCHD meetings with supervisor to receive “pass downs”. Ensure client records are secured, and confidentiality is maintained.

MINIMUM QUALIFICATIONS:
Active licensed Registered Professional Nurse in accordance to with F.S. 464 or eligible to practice nursing in accordance with F.A.C. 210-6.27 and have one year of professional nursing experience.
A minimum of 6-month immunization experience.
May be required to work during or beyond normal work hours or days in event of an emergency. Emergency duty required of the incumbent working in special needs or Red Cross Shelters, or to perform other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
At emergency duties, incumbent is expected to perform basic nursing skills such as: vital signs, health assessment, ensure safety and security for clients, infection control measures, hygiene care, wound care (if needed), etc. We provide skill refresh training.
Willing to work at NCF Englewood or NCF Bithlo locations. Incumbent could request a specific location, which will be taken into consideration: however, this request is not a guaranteed placement of location.
Required to work 6 Saturdays of the year.
This position requires daily in-office or field work.
