
Responsible for assisting the needs of the Home Care Coordinator which include communication with Personal Care Contracted Companies and DME Contracted Companies. Assists in Setting up Personal Care Services in the home and ordering and tracking DME and supplies. Assists in the coordination of care between the families, personal care companies and transportation. Performs telephone audits with the participants receiving personal care in the homes.
Experience - One year working with the frail elderly.
Education - High School Diploma
Training - Current Certified Nursing Assistant Certification in the State of Louisiana
Special Skills - Computer Skills
BLS & First Aid Certification
