Practice Highlights: Saint Alphonsus Medical Group (SAMG) is a physician led and accountable 650+ provider multi-specialty group practice that has an excellent opportunity for an Advanced Practice Provider (NP/PA) to support two complementary areas of care: the Anchor Clinic and the Saint Alphonsus Health Alliance’s Pre-Visit Planning (PVP) chart preparation workflow. This Advanced Practice Provider role will be based within the brand-new Caldwell Health Plaza.
The Caldwell Health Plaza opened last fall and is now a vibrant, fully operational, state-of-the-art 60,000+ sq ft facility. The Health Plaza serves the community members seeking wellness, diagnosis, treatment, and rehabilitation in a convenient and accessible environment, located directly off I-84. With comprehensive healthcare services offered for the whole family, it offers a multitude of services in one convenient location including: Extended Hours Urgent Care, Family Medicine, Pediatrics, Occupational Medicine, Diabetes Care & Education, Sleep Medicine, Orthopedics, Heart Care, Physical Therapy, Occupational Therapy, Podiatry, MFM, ENT and onsite Lab, X-ray, and ultrasound.
Community: Caldwell, Idaho, population of almost 60,000, lies west of Boise and is located at the western end of the Boise metro area; Located along Interstate 84, the area boasts excellent transportation corridors in all four directions.
