As an Advanced Practitioner at CapitalCare Family Practice Slingerlands, you will provide full-spectrum family medicine care alongside experienced physicians and Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and strong patient relationships, with modern tools such as telemedicine and a secure patient portal to support coordinated care and patient access.
Conduct comprehensive patient evaluations and manage acute and chronic conditions
Order and interpret diagnostic studies, prescribe medications, and coordinate specialty referrals as needed
Provide preventive care, wellness visits, and patient education across the lifespan
Collaborate closely with physicians, Advanced Practitioners, nurses, and support staff to ensure continuity of care
Maintain accurate and timely clinical documentation in accordance with organizational standards
Care across the lifespan – serving pediatric, adult, and geriatric patients in a community-based family medicine setting
Early morning weekday hours, offering convenient access for patients and structured scheduling for providers
Telemedicine visits and secure patient portal, supporting flexible access and ongoing patient communication
Established collaborative team of physicians and Advanced Practice Providers serving a strong patient panel
