Community Care Physicians – Community Care Weight Management and Metabolic Care
Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join Community Care Weight Management and Metabolic Care, a specialized, multidisciplinary program focused on helping patients achieve sustainable weight loss and improved metabolic health.
As an Advanced Practitioner in the Weight Management and Metabolic Care program, you will provide evidence-based care for adult patients with obesity, Type 2 diabetes, and related metabolic conditions. You will work within a highly collaborative, multidisciplinary team—including physicians, dietitians, behavioral health specialists, and clinical pharmacists—to deliver personalized treatment plans that integrate lifestyle modification, medical therapy, and ongoing support.
