Community Care Physicians – CapitalCare Family Practice River Road
Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Family Practice River Road, a well-established primary care practice providing comprehensive care to patients of all ages and committed to coordinated, high-quality, patient-focused care.
As an Advanced Practitioner at CapitalCare Family Practice River Road, you will deliver full-spectrum family medicine services in a collaborative, team-based environment. You will practice to the full extent of your license with strong physician support and access to a wide network of specialty care, all within a setting designed to promote work–life balance and long-term professional satisfaction.
Conduct comprehensive patient evaluations and manage acute and chronic conditions
Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed
Provide preventive care, health maintenance, and patient education
Collaborate closely with physicians, nurses, and support staff to ensure continuity of care
Maintain accurate, timely clinical documentation in accordance with organizational standards
Care across the lifespan – serving pediatric, adult, and geriatric populations
Convenient health park location, co-located with multiple specialties including imaging, lab, pulmonary, neurology, oncology, and more—supporting seamless referrals and coordinated care
Early morning hours, enhancing access for patients and offering scheduling flexibility for providers
Comprehensive on-site services, including preventive care, occupational health services, and diagnostic testing
Established, collaborative team of physicians and Advanced Practitioners within a high-demand community practice
Compensation: Starting at $125,000
CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.
