• $10,000 Sign-On Bonus
• $120,000 to $140,000+ Base Salary, highly negotiable based on experience

This organization is a leading national provider of integrated medical services, caring for individuals from post-hospitalization to home, including in skilled nursing facilities (SNFs). Their providers bring quality medical care to the geriatric population, making a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve.

POSITION DESCRIPTION:

Serve as a primary care provider for geriatric patients located in a skilled nursing facility. A low volume of patients are seen per day, allowing ample time to focus on all needs and to provide proper attention and support.

Overall duties of the role include, but are not limited to:

• plan, direct, and evaluate medical care
• examine and treat chronic illnesses
• examine and treat urgent illnesses
• order and interpret laboratory and diagnostic testing
• collaborate with caregivers and other healthcare professionals
• work closely with the interdisciplinary care team
• make referrals as necessary

The practice uses an EHR on a mobile-based platform and providers are supported by team members who assist with scheduling, orders, referrals, etc.

Providers share on-call responsibilities, which is low volume phone triage. Providers are on call no more than 3 nights per month and flexibility in that schedule is offered.

REQUIREMENTS

• Virginia NP license
• DEA
• 1+ year advanced practice experience
• 1+ year experience in relevant settings (facilities, home care, geriatrics, etc.)
