Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs.
The Cleveland Clinic Center for Functional Medicine is looking to add an Advanced Practice Provider who is experienced in Functional Medicine and certified by The Institute for Functional Medicine (IFM). This provider must be able to integrate this patient care approach with the reputation and integrity of the Cleveland Clinic. This Advanced Practice Provider will focus on the Functional Medicine team approach to provide excellent patient experiences and deliver innovative, value-based care at the Cleveland Clinic.
Key functions will include:
-Building a patient-focused functional medicine practice
-Building an internal and external referral network that results in chronically ill patients coming to the Cleveland Clinic Center for Functional Medicine
-Providing feedback to Medical Director and Practice Administrator on effective mechanisms to improve patient flow, patient care, EMR documentation, diagnostic testing options, and nutraceutical therapeutics
Job Activities
-Provide chronic disease care from a Functional Medicine perspective by interviewing, assessing, and treating clinical patients, meeting their medical needs, and improving their quality of life
-Perform a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician
-Conduct thorough medical histories
-Initiate appropriate lab, radiology tests or other special tests required for evaluation of illness
-Order, interpret and evaluate laboratory test and other diagnostic studies to assess patient's clinical problems and health care needs
-Ability to work with autonomy within applicable state law under direction of supervising physician
-In collaboration with the physician, develop a treatment plan along with the patient and significant others
-Perform and interpret, at least to the point of recognizing deviations from normal, common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board
-Provide information and answer routine questions of patients regarding their disease, including preventive health maintenance topics such as diet, weight, smoking and topics related to Functional Medicine, with basic recommendations
-Educate patients regarding their disease, treatments, related drug and treatment side effects and hazards
-Assist with record keeping, complete appropriate patient medical chart, transcribe lab data and routine correspondence, as directed and reviewed by physician
-Ensure continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary
-Perform research follow-up studies as directed by the supervision of a physician
-Assist in data analysis
-Participate in quality monitoring thru the review of records and treatment plans for patient outcomes on a periodic basis to assure quality care
-May serve as clinical preceptor for PA students and other health professionals
-Demonstrate a high standard of moral and ethical behavior, compassion, professionalism and a commitment to excellent patient care
Specific Characteristics and Skills Required
• Functional Medicine Certification through The Institute of Functional Medicine (IFM)
• Excellent analytical and decision making skills
• Ability to work effectively and efficiently with diverse team members, caregivers, students, and patients
• Superior communication skills, emotional intelligence, and the ability to function in a rapidly changing environment

Effective 05/01/2014: MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
Graduate of an accredited APRN program.
If in neonatal care service area Master’s Degree is required upon hire.
PA
Graduate of an A.R.C. approved Physician Assistant Program.
Master’s degree in Physician Assistant Studies and/or higher degree obtained from a program recognized by their practicing states Licensing Board.
