We are seeking a full-time Nurse Practitioner or Physician Assistant to join our community-focused outpatient family medicine clinic in Dewey-Humboldt, AZ This is an excellent opportunity for providers who value long-term patient relationships, a supportive team culture, and a predictable weekday schedule With no nights, weekends, or on-call duties, this role allows you to focus on high-quality, patient-centered care

Compensation & Benefits

• Salary: $120,000 – $150,000 annually (based on experience)

About the Clinic

Our Dewey-Humboldt clinic is part of a mission-driven primary care network serving both rural and suburban communities across Arizona. Key Responsibilities

• Perform annual wellness exams, preventive screenings, and Medicare visits
• Diagnose and manage acute conditions and minor injuries
• Provide chronic disease management (diabetes, hypertension, COPD, etc.)
• Order, interpret, and follow up on diagnostic tests and imaging
• Educate patients on medications, treatment plans, and healthy lifestyle practices
• Maintain accurate EHR documentation with synchronous scribe support
• Coordinate referrals and manage transitions of care
• Participate in clinical quality initiatives and practice improvement efforts

Qualifications

• Active and unrestricted Arizona NP or PA license
• Current DEA registration or ability to obtain
• Credentialing eligibility with Medicare, Medicaid, and commercial insurers
• No history of malpractice or disciplinary action
• Graduate of an accredited U.S.-based NP or PA program
• (Note: Puerto Rico-based programs are not accepted)
