We are looking for a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) for our Senior Living Community in Northfield, MN. The provider delivers primary care to senior community residents, focused on preventive care, chronic-condition management, and care coordination. This is a full-time, in-person position with predictable weekday hours and strong work-life balance.

Qualifications:

• Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Minnesota, in good standing

• Active DEA registration

• 1+ years of clinical experience; senior community, primary care, or geriatrics preferred

• Familiarity with EMR systems and care documentation

• Excellent communication with residents, families, and community staff

Responsibilities:

• Conduct routine primary care visits and chronic-condition management for community residents

• Lead preventive care including immunizations, screenings, and falls/risk assessments

• Develop care plans and partner with families on health goals

• Coordinate transitions when residents need higher levels of care

• Engage with families on goals of care and advance directives
