We are seeking a dedicated and compassionate Nurse Practitioner to join our Charlottesville, VA team on a part-time basis. This role offers a meaningful opportunity to provide high-quality, patient-centered care within post-acute, long-term care and assisted living settings. The successful clinician will work collaboratively with facility staff and multidisciplinary teams while maintaining clinical autonomy and flexible scheduling.

Provide comprehensive, patient-centered medical care in post-acute, long-term care and assisted living environments
Conduct admission, routine and urgent assessments; develop and implement treatment plans
Prescribe and manage medications within scope of practice and in accordance with Virginia regulations; perform medication reconciliation and reviews
Collaborate with nursing staff, advanced practice colleagues, therapists and interdisciplinary teams to coordinate care and transitions
Respond to on-site clinical issues, participate in care conferences and support safe discharges
Document patient encounters accurately and promptly in the Electronic Medical Record (EMR), ensuring clear communication of plans and follow-up
Participate in quality improvement activities, case reviews and infection prevention initiatives as required
Provide education and clinical guidance to facility staff, residents and families as appropriate
Master’s degree or higher as an advanced practice nurse (NP) from an accredited institution
Active, unrestricted Virginia NP license and current DEA registration
Prescriptive authority and experience practicing in post-acute, long-term care, geriatrics or related settings preferred
Proficiency with Electronic Medical Record systems and timely, clear documentation practices
Excellent clinical judgement, strong communication skills and ability to work both autonomously and collaboratively
Culturally sensitive, patient-centered approach and commitment to dignity in care
